Health Risk Assessments - Improving Completion Rates

 

HRAs are critical tools for identifying member needs and barriers that adversely impact care quality, care coordination and health outcomes, and have become core performance measures for payers – especially MA and Dual-Eligible Special Needs Plans.  Integra’s Health Risk Assessment (HRA) program offers payers and providers a solution to increase HRA completion rates that can be rapidly implemented to supplement existing efforts.

Integra deploys field-based Community Coordinators to find and engage members who are unresponsive to traditional outreach campaigns. We typically achieve success completing HRAs with over 40% or members referred to us as ‘unreachable’, improving if not doubling HRA completion rates. Our staff works one-on-one with members in their own communities to facilitate scheduling and completion of the HRA.
